How can i ask her out?

I meat this girl in school on March or April. But i meat her only to get to know where she's from and her name but forgot where she is from. So in other words i don't know her that much. I want to go out with her and i am scared to ask her out. But i think she kind of like me don't know she alway come to me when she see me to say hello and kiss me in the chist or i go to her. She say i dress nice/cool and she say something about my hair that i did't quite get but it was something like is to soft when she put her hand on it. I could asked her phone # on the last day but i was too scared to do it. So PLEASE HELP ME OUT HERE so when i get to see her again do it right
