My girlfriend and I broke up a while back, but I feel like it was my fault. I really love her and wish she would take me back.

First, it can never be 100% your fault. It's nice of you to shoulder the blame but unless you face this completely realistically, you are just going to fail again. You BOTH are to blame because you BOTH did not work on the relationship fully. A relationship needs 2 people that can work with each other, find solutions to problems, and work as a team. The fact that you guys gave up instead of trying to fix things indicate that something pretty serious had caused a disconnect in your underlying friendship.
