troubled with a coworker

I work at a place where i can interact with many people, i met a guy at work, for a few weeks he has been asking me out and then telling me to call him when i get to the place where im suppost to meet him. i go, call him and tell him im there and he never shows, im never alone when i go to the places where im suppost to meet him so im not sitting there alone, but when i see him at work, he makes an effort to say he is sorry about not showing and we go off alone and spend time together.
My question is how do i tell him that its not okay to never show where he is suppost to and that this hanging out at work is not okay either.

If it is difficult to do in person, give him a note or send him an email.
